The SDBON will release open public record information in an Excel file format on actively licensed individuals.
- Email a written request to the Board indicating licensure list(s) requested
- A fee of $100 will be assessed per license type requested
- An invoice will be issued for payment.
- Payment must be received prior to a list being released
